I would get a second opinion from another vet. Get a skin scraping test done. I am also a huge believer that all dogs can use a top dressing food supplement for great skin and coat. When your dogs food alone is not working to keep weight on your dog I highly recommend add a digestive enzyme to each meal.
Products like Prozyme, K9 Power Digest Forte, Petenzymes.com Total-Zymes.

If your using a crate then I agree with using high value food treats with the command kennel. Stuffed Kong toy works well too. From what I understand of this condition it is a bacteria picked up from the food / water bowsl. Especially on plastic bowls. Stainless Steel bowls are easier to disinfect and helps eleminate this condition. Cleaning the dogs affected area with Hydrogyn Peroxide should work well to dry up the "zits"...

Punishment is not really the answer for digging. The bottom line is the pup needs more mental and physical exercise. Working on obedience and exercise keeping age in mind before you leave the dog alone in the yard may help a lot. I would leave stuffed KONG toys for her to work on that will help too....

If that's what the breeder weaned the pups onto I would stick with it as long as you don't over feed and cause the pup to get diarrhea. That is pretty rich food for some dogs. Very high fat.

Some dogs hate confinement. When you put your pup in the crate I would also put in a food stuffed KONG toy. Should help the anxiety. You can also try using a few drops of Rescue Remedy on his tounge. Google Rescue Remedy for more info.
