That is pretty unusual for an adult dog. He should get a thorough check-up to see if there is an underlying problem. Demodex is a secondary problem. The vet did a scraping and concluded it was Demodectic Mange? It does look like it. Best also to feed better, more protein coming from animal sources ...

Try soaking in Epsom salt and hot water solution, at least once a day. There may be some foreign material still in there, but it can be very hard to find and often you end up causing more damage digging around in there. If he's walking on it I would just be patient for now. Easy for me to say, I know.

They need their hormones to be considered "mature". That doesn't happen until atleast 2yrs old. If the dog has had 2 heat cycles, trust me, the hormones are present in full force. a positive test just means your dog has antibodies against the bacteria that causes lyme disease in its system. and antibodies mean that the dog has been exposed to that bacteria, not that they actually have an active infection.

looks like it would be very easy to remove, maybe even with local anesthetic so the dog wouldnt have to be put under. could just be a simple skin tag. I doubt it would come back after removal.

ask your vet if he has a recipe for a homemade diet that would be easy on the kidneys (basically low protein, low phosphorus, etc). might need to contact a nutritionist at a university. also might ask about a product called Azodyl.

Lepto is one of those things that doesnt always get diagnosed definitively, so the statistics regarding reported cases may be misleading. My opinion is vaccinate your dog. Everything that pees can carry lepto.

you might ask your vet about doing the 2 shot protocol (2 shots 24 hrs apart). That should reduce the cost.
